The Lorient left-back has been linked with a move to Anfield but he looks set to join Dortmund for £9.5m (?12m) after undergoing a medical yesterday.
Dortmund are waiting for the all-clear from club doctor Markus Braun, according to German paper Kicker, before getting the deal over the line.
Guerreiro is expected to sign a four-year deal until 2020 with the German giants.
The 22-year-old, who is currently with Portugal at Euro 2016, impressed in 41 appearances for French side Lorient last season.
It was reported last month that Liverpool had made an approach for Guerreiro while TransferMarketWeb recently claimed the Reds were in talks with his representatives.
Lorient vice-president Alex Hayes confirmed Jurgen Klopp's interest.
He said: "No one wants to see Raphael leave, especially the president.
"He will only be transferred if a club makes an offer that we can't refuse.
